Letter of Invitation for Visa Purposes

Amputation Prevention Symposium (AMP) is proud to welcome international participants for this educational activity. Recognizing that travel visas may be necessary and the process time is consuming, please be sure to allow enough time for the process. Click here for detailed information.

A letter of invitation for participation may be provided, upon request, as part of the visa process. This invitation represents no intent by AMP to provide financial support, nor does it serve as a guarantee in any respect to the visa application process. Letters of invitation will be sent only to registered attendees. To request an invitation letter, please contact Erica Paik at epaik@hmpglobal.com.

Amputation Prevention Symposium (AMP) Disclaimer

AMP, its contractors and attending news media may be photographing or videotaping scientific sessions and events at the meeting. By attending AMP, attendees acknowledge these activities and agree to allow their image to be used by AMP in association publications, on the AMP website, and in marketing and promotional materials. Attendees at AMP waive all claims against AMP for any liability resulting from these uses.

 

Cancellation Policy

Please note the cut-off date for cancellation is June 22, 2018. All cancellations must be received in writing and postmarked by that date. Full registration (less a $150 processing fee) will be refunded only to cancellations received in writing that are postmarked by the above date. No refunds will be issued after June 22, 2018.

There are no exceptions to these policies. Payments may not be applied toward tuition for future AMP conferences, or other meetings or products offered by HMP or NACCME.

 

Children

Under no circumstances will children under the age of 18 be allowed in the exhibit hall or educational session rooms at any time.

 

Session Seating

Seating is limited for all session rooms and is available on a first come, first served basis.

 

Liability Statement

Definitions:

HMP is the owner of AMP. HMP and AMP are known collectively in this Statement as AMP.

I agree and acknowledge that I am undertaking participation in AMP events and activities as my own free and intentional act, and I am fully aware that possible physical injury might occur to me as a result of my participation in these events. I give this acknowledgement freely and knowingly and that I am, as a result, able to participate in AMP events, and I do hereby assume responsibility for my own well-being and safety.

AMP plans to take photographs and video material at AMP group events and reproduce them in AMP educational, news or promotional material, whether in print, electronic or other media, including the websites. By participating in AMP group events, you grant AMP the right to use your name, photograph and biography for such purposes. All postings become the property of AMP. Postings may be displayed, distributed or used by AMP for any purpose.

Furthermore, I acknowledge AMP’s right to crop, splice, treat and edit any photographs taken of me are at the AMP's sole discretion. I waive my right to inspect or approve the finished product, now and in the future, whether that use is known or unknown to me.

I hereby agree to release, defend, and hold harmless AMP and its agents or employees, including any company publishing and/or distributing the finished product in whole or in part, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion, or in the taking, processing, reduction or production of the finished product, its publication or distribution.

 

Copyright Notice

Companies that submit artwork or upload or link any material (including but not limited to images, text, documents and/or video files) to the website are solely responsible for the content of such material and for gaining required permissions under copyright, trademark, trade secret or other applicable law for the use of such material. (Caution: Many websites purchase artwork from stock photo suppliers for the expressed website use or from hired photographers who have only supplied one-time rights.) By submitting any such material, the submitting company represents and agrees that (i) it has obtained the necessary permissions referenced above, (ii) any such material is not likely to defame and does not invade the privacy of any person, and (iii) it will defend, indemnify and hold harmless AMP, its affiliates and their respective directors, employees and agents (including, without limitation, its publishing partner or any agent acting on its behalf) from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including reasonable attorneys' fees) resulting from the submitting company's breach of the obligations set forth in this paragraph.

 

Acceptance

By registering for and attending AMP group events, I hereby agree that I accept the terms stated above, and indemnify and hold harmless AMP, its affiliates and their respective directors, employees and agents (including, without limitation, any agent acting on its behalf) from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including reasonable attorneys' fees) resulting from attendance at AMP events.
